Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services and programs addressing the social determinants of health such as housing and employment. Aspire has health centers in five Central Indiana counties serving Hoosiers of all ages and walks of life.

 

Position Summary 

The Supportive Services for Veterans and Families (SSVF) Case Manager will have certain direct responsibilities key to the success of the InterCare SSVF Program to ensure that Veterans and their families obtain and maintain permanent housing, as well as needed coordination and linkage to supportive services. The SSVF Case Manager is responsible for working with the Veteran and their families in order to locate, obtain and maintain permanent housing. In addition, the SSVF Case Manager will work with the Veteran, their family and the Peer Mentor as a team in order to determine and coordinate additional support as needed.  This position requires travel to support individuals across multiple counties, and work hours may be contingent upon grant funding.  

 

Education and Experience

  • High School Diploma or Equivalent required; Bachelor's degree in social work, health care, or related field preferred from a college/university accredited by the US Department of Education 
  • A minimum of two (2) years case management experience required or relevant military experience can be substituted for education and/or case management experience

  • Must be able to demonstrate accuracy and thoroughness and the ability to problem solve and utilize critical thinking skills
  • Must be able to demonstrate the effective use of Aspire’s Electronic Health Record (EHR)
  • Ability to work well under pressure in a fast-paced, time sensitive environment with shifting priorities and multiple deadlines
  • Must possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization
  • Must have reliable transportation to oversee programming and/or attend trainings and/or meetings
