Now interviewing for a Community Health Worker - Transportation position for Hamilton County 

Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services and programs addressing the social drivers of health such as housing and employment. Aspire has health centers in four Central Indiana counties serving Hoosiers of all ages and walks of life.

Position Summary

Community Health Worker's (CHWs) are flexible members of a health care team who work with vulnerable populations to improve health outcomes.  The goal of the CHW is to improve the health of the community by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy.   The Community Health Worker - Transportation will provide and/or coordinate transportation services to clients and employees and coordinate Aspire fleet maintenance. They will also maintain contact with clients by picking them up at their homes and making telephone calls, promote a medical home model where the CHW, healthcare team and other resources interact to meet the client's needs for services and education and provide referrals as needed. 

 

Education and Experience

  • High School Diploma or equivalent required, Bachelor’s Degree in social work, psychology, counseling or related human service field from a college/university accredited by the US Department of Education preferred
  • One (1) year of related driving experience required
  • Must be able to drive in varying weathering conditions required
  • 15 passenger van or bus driving experience and basic motor vehicle knowledge (checking oil, tires for air and tread wear, etc.) strongly preferred

 

Certification

  • Must be able to achieve and maintain Community Health Worker certification within 12 months of hire
