Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services and programs addressing the social determinants of health such as housing and employment. Aspire has health centers in five Central Indiana counties serving Hoosiers of all ages and walks of life.

 

Position Summary

 

Aspire Indiana Health’s FQHC is looking for someone who is excited to be a part of their patient care team in an integrative healthcare setting. The Behavioral Health Consultant (BHC) will work in a fast paced environment and provide comprehensive care to medical and psychiatric patients, assisting patients to improve their overall health and wellness in a primary care/psychiatric setting.  Unlike traditional therapy treatment, the BHC provides the choice to the patient on whether they would like to follow up on the goals they set at their next provider appointment, schedule a follow up directly with the BHC or access the service if and when they feel they need it again

 

This dynamic clinical position is full-time  and involves “in-the-moment” collaboration with the FQHC patient care team members made up of medical and psychiatric providers, medical assistants, nurses and administrative staff. The BHC role is primarily onsite, allowing for collaboration with team members and the ability to immediately engage face to face with patients through warm hand offs. However, Aspire Indiana Health understands the importance of flexibility, and offers the option to work remotely as needed based on both the clinic and employee needs.

 

Specific responsibilities include but are not limited to:

 

  • Huddling with providers at the beginning of each clinic day to discuss health behavior needs of the patients on their schedule
  • Receive warm hand offs from medical and psychiatric providers to address identified health behavior or psychiatric concerns/barriers
  • Providing concise, yet comprehensive, assessment of behavioral health issues 
  • Working with patients to set specific and attainable goals related to these identified health or psychiatric related barriers to overall health
  • Linking patient to internal or external resources to meet their identified goals when needed 
  • Assessing risk and planning for safety for high risk patients

 

Candidates with minimal integrative health care experience are still encouraged to apply! As a new hire, the BHC will be provided ample time to shadow the BHC’s and FQHC providers’ appointments in order to gain knowledge and confidence of how the service is delivered.  In addition, they will be guided through a thorough education that outlines the BHC role and responsibilities with close access to a supervisor to answer questions and concerns.  This is a great opportunity to become a skilled integrated care provider!

 

Education and Experience

  • Master’s degree in Social Work, Psychology, Counseling or other mental health related field or a Doctoral degree in Psychology from a college/university accredited by the US Department of Education required 
  • Three (3) years clinical experience required
  • Knowledge of and experience with providing brief evidence based practices required
  • Experience in an integrated care setting preferred

 

Licensure

  • Must be recognized as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Licensed Psychologist (HSPP) by the State of Indiana
