Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services and programs addressing the social drivers of health such as housing and employment. Aspire has health centers in four Central Indiana counties serving Hoosiers of all ages and walks of life.

Position Summary

The Supervisor-  HOPE (Health, Outreach, Prevention, Education) will provide direct supervision and oversight of the Case Managers of Team HOPE, providing HIV, Viral Hepatitis, and STI services and prevention. This position will provide support to staff in multi-county regions, and to the Manager in monitoring and evaluating all program services and administrative tasks provided through the HOPE program.

  • Identify and direct client interactions throughout the service regions
  • Assign tasks, provide education and support to ensure high quality and efficient outreach activities
  • Work with other team members effectively to assure optimal client outreach and engagement
  • Provide clients with diagnosis-related patient education on self-management of healthcare conditions in conjunction with the healthcare team 
  • Supervise team members to ensure the team maintains contact with clients by conducting home visits and making telephone calls
  • Build trusting relationships through active listening
  • Supervise the Team HOPE activity to assess health and social needs as identified by patient priorities 
  • Promote a model where the Team HOPE staff, healthcare team, and other resources interact to meet the patient’s needs for services and education 
  • Create and promote a work climate that emphasizes collective team performance and demonstrates Aspire’s core values while supporting an employee empowered culture and nurturing ideas
  • Screen, interview, and make recommendations to hire new staff
  • Arrange appropriate on-boarding, training, and performance reviews for staff
  • Perform and evaluate staff performance and competency evaluations within required and assigned time frames and standards  and provide regular coaching and feedback

Education/Experience

  • High school diploma or equivalent required, Associate’s or Bachelor’s degree preferred 
  • Two (2) years of comparable work experience required
  • Previous experience in a supervisory capacity preferred
  • Knowledge and application of utilizing social service systems (Medicaid, Social Security, Corrections, etc) highly preferred
  • Community Mental Health experience highly preferred 
  • Must possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization
  • Must have reliable transportation to oversee programming and/or attend trainings and/or meetings
